Trinity Healthcare of Logan is proud to announce participation in the Centers for Medicare & Medicaid Services (CMS) Quality Improvement Program (QIP) for 2025–2030 through the Mid-Atlantic Quality Innovation Network – Quality Improvement Organization (QIN-QIO), led by IPRO in collaboration with Qlarant.

This national initiative brings healthcare organizations together to improve patient outcomes, strengthen care coordination, and advance quality across the healthcare continuum.

What Is the CMS Quality Improvement Program?

The CMS Quality Improvement Program is a federal initiative designed to support healthcare providers in improving the quality, safety, and value of care delivered to patients and residents.

The program is implemented through regional Quality Improvement Organizations (QIOs), which work directly with healthcare providers to identify opportunities for improvement and implement evidence-based strategies.

For the Mid-Atlantic region, this work is coordinated through the CMS QIN-QIO, led by IPRO in collaboration with Qlarant.

Through this partnership, participating healthcare providers gain access to:

  • Evidence-based quality improvement resources

  • Data analysis and performance benchmarking

  • Clinical best practice guidance

  • Educational opportunities and collaborative learning networks

  • Tools to support measurable improvement in care outcomes

These resources help facilities like ours continually refine and strengthen the care we provide.

Supporting Continuous Quality Improvement

Quality improvement in healthcare is not a one-time initiative—it is a continuous process of learning, evaluating outcomes, and implementing better practices.

Participation in the QIP supports our facility’s commitment to:

  • Enhancing resident safety and quality of life

  • Improving clinical outcomes and care coordination

  • Supporting workforce development and staff education

  • Strengthening communication across the care team

  • Promoting data-driven decision-making

A Commitment to Residents and Families

At Trinity Healthcare, our mission is to provide compassionate, high-quality care to the residents and families we serve. Participation in the CMS Quality Improvement Program reinforces our commitment to maintaining high standards and continuously improving our services.

As Administrator Jordan Gibson explains:

“Quality care is not a one-time achievement—it is a continuous effort to learn, improve, and grow. Programs like this help ensure we remain focused on best practices and measurable outcomes across the continuum of care.”

Through collaboration with national quality organizations and a dedicated team of healthcare professionals, Trinity Healthcare of Logan continues to build a culture where quality care is not only delivered—it is felt in every interaction.
